Forms & Documents

Queensland changed the Building Regulation 2006 to the Building Regulation 2021 on (1 September 2021), this included major changes to the required inspection forms and processes. There are currently three inspection certification forms (Forms 12, 16, and 15) that we could be required, depending on the building's class, who is delivering the certificate, and what they are certifying.

Additionally, we have collected the most common forms used in the fire industry for ease of access.

Form 12

This form is to be used for the purposes of sections 74 and 77 of the Building Regulation 2021

This form is to be used by an appointed competent person for the purposes of section 10

This form is to be used for the purposes of section 10 of the Building Act 1975 and section 53

Form 71

This form is to be used for the purposes of commissioning water based fire safety installations

Form 72

This form is to be used for the purposes of maintenance to water based fire safety installations

Occupier's Statement

This form is to be provided on an annual basis with regards to the upkeep of the fire services
